Welcome to the Southeastern Ontario Palliative End-of-Life Care Network (www.seopalliativeeolcare.ca) website.
The Southeastern Ontario Palliative and End-of-Life Care Network is a vehicle that facilitates people and resources coming together to improve the system of palliative care for residents of Southeastern Ontario. It is broad-based, open and representative of the organizations and individuals that coordinate and deliver palliative and end-of-life care.
What is Hospice Palliative Care?
Hospice palliative care is aimed at relief of suffering and improving the quality of life for persons who are living with, or dying from, advanced illness or are bereaved.
Hospice palliative care is appropriate for any patient and/or family living with, or at risk of developing, a life-threatening illness due to any diagnosis, with any prognosis, regardless of age, and at any time they have unmet expectations and/or needs, and are prepared to accept care.

Vision Statement
Southeastern Ontario residents and their families will have access to exemplary Palliative and End-of-Life Care when needed. Care will be provided in the most appropriate setting through an interdisciplinary program of care which focuses on quality of life, control of pain and symptoms and attends to the psychological, spiritual experiences of individuals and their families in adapting to illness, preparing for life departure and bereavement.1
The Network will use the terms “palliative” and “end-of-life” care together or “palliative care” alone in order to maintain focus on the broader continuum of care as described by the CHPCA.
