Who We Are

In October 2005 the Ministry of Health and Long-Term Care (MOHLTC) announced the Provincial End-of-Life Care Strategy. The goal of the strategy is to promote comprehensive, consistent and high quality end-of-life care across the province. The objectives include:
  • Shifting care of the dying from acute care settings to appropriate alternate settings of individual preference (e.g. home, long-term care home, residential hospice)
  • Enhancing and developing client-centered and multidisciplinary service capacity in the community
  • Improving access, coordination and consistency of services and supports across the province

In support of the strategy organizations and individuals from across the province came together to form 14 Regional Hospice Palliative Care/End-of-Life Care Networks as mandated by the MOHLTC. Through their membership, Networks allow for a broad recognition of the many issues that challenge the health-care system in providing palliative and end-of-life care to individuals with life-threatening illnesses and their families. Networks assist with the ongoing implementation of Ontario's End-of-Life Care Strategy by promoting a consistent and efficient response to palliative care issues through the adoption of best practices, knowledge exchange and coordination.

What We Do

The roles identified for End-of-Life Care Networks by the MOHLTC include:
  • Broad system design for palliative and end-of-life care
  • Coordination and integration at a system level
  • Monitoring and assessment of community needs and
  • Promotion of service innovations
To achieve these roles the Network provides a valuable local forum to: Facilitate the exchange of information;
  • Improve system coordination, decrease duplication and increase efficiencies
  • Promote consensus building and consistency in best practices
  • Promote, implement and operationalize the Canadian Hospice Palliative Care Association Norms of Practice and
  • Participate in evaluation processes

Mission Statement

Inclusive of and building on the strengths of current networks and organizations that coordinate and deliver care, the Southeastern Ontario Palliative and End-of-Life Care Network will provide leadership and structure to facilitate the development of plans and processes to optimize regional palliative and end-of-life care outcomes in Southeastern Ontario.

Mandate

Inclusive of and building on the strengths of current networks and organizations that coordinate and deliver care, the Southeastern Ontario Palliative and End-of-Life Care Network will provide leadership and structure to facilitate the development of plans and processes to optimize regional palliative / end-of-life care and outcomes in Southeastern Ontario.

Objectives

  • To plan and design a system of exemplary palliative and end-of-life care
  • To facilitate coordination and integration of palliative and end-of-life services at a system level
  • To assess and monitor community needs
  • To promote identification of and implementation of service innovations

Guiding Principles

The Southeastern Ontario Palliative and End-of-Life Care Network will respect the autonomy of member organizations and individuals while fostering opportunities to work together on priorities that affect palliative and end-of-life care across the continuum and across care settings. In doing so, the Network will adopt and adapt the principles and norms of practice outlined by the Canadian Hospice Palliative Care Association in a Model to Guide Palliative Care (March 2002). The principles describe all aspects of palliative and end-of-life care, specifically:1
